How to Create and Use Lead Capture Forms in myCRMSIM

Overview
Lead Capture Forms let you collect contact details directly from your website. When a visitor submits a form, myCRMSIM automatically creates or updates their contact record, records the submission, and can trigger an automation workflow for instant follow-up — such as a thank-you SMS, a nurture sequence, or a tag applied to the contact.
What happens when a visitor submits a form:
  1. Visitor fills in their details on your website (name, email, phone, and any custom fields)
  2. A contact is created (or updated) in your workspace
  3. The submission is recorded against the contact with a timestamp and the page URL
  4. Any connected automation workflow fires automatically
  5. You can view all submissions inside myCRMSIM under the form's Submissions tab
Step 1 — Go to Lead Capture
  1. Log in to myCRMSIM at app.mycrmsim.com
  2. In the left sidebar under Workspace, click Lead Capture
  3. The Forms tab is selected by default — this is where all your forms live
  4. Click + New Form to open the form builder

Step 2 — Build Your Form Fields
The form builder opens with three default fields already added: First Name, Email (required), and Phone Number. You can customise these and add more.
Adding Fields
Click Add Field to see all available field types:

Field Type

What it Captures

First Name

Visitor's first name

Last Name

Visitor's last name

Email

Email address (validated format)

Phone Number

Mobile number (validated format)

Message / Text

Free-text input or textarea

Select / Dropdown

A choice from a predefined list

Checkbox

A yes/no tick box

Custom Fields

Any custom contact fields you have defined in your workspace

Note: Every form must include at least one of Email or Phone Number. You cannot save a form without at least one of these — they are how myCRMSIM identifies and deduplicates contacts.
Field Options
For each field you can:
  • Add a label and placeholder text
  • Toggle Required on or off
  • Drag fields to reorder them
  • Click the remove icon to delete a field
Fields already added to the form disappear from the Add Field menu to prevent duplicates.

yoursite.com — leave empty to allow all

Tip: Use tags to segment your leads. For example, a form on your pricing page could tag contacts as pricing-interest so you can target them with a specific follow-up sequence.

Step 5 — Get the Embed Code
  1. On the Forms list, find your form and click the embed icon (< >) in the Actions column
  2. A modal appears with two options:
Option A — Embed on your website (recommended)
Copy the script snippet and paste it into the <head> or just before the closing </body> tag of your webpage:
HTML
<script src="https://backend.mysmsgateway.app/embed/forms.js" data-form="YOUR_FORM_ID" async></script>
The form will render automatically inside an iframe on your page. The iframe height adjusts to fit the form content — no scrollbars, no empty space.
Option B — Share a direct link
Copy the direct URL (e.g. https://backend.mysmsgateway.app/f/YOUR_FORM_SLUG) and share it directly — by email, SMS, or social media. The form opens as a standalone page.
Note: Each form has a unique ID in the embed code. Do not mix up embed codes from different forms.

Step 6 — Test Your Form
Before going live, test the form end-to-end:
  1. Open your website (or paste the embed code into a test HTML page)
  2. Fill in a test name, email address, and phone number
  3. Click the submit button
  4. Check that:
  • The thank-you message appears (or the page redirects) ✅
  • A new contact appears in myCRMSIM → Contacts ✅
  • The submission appears in myCRMSIM → Lead Capture → Forms → Submissions ✅
  • The contact's Notes section shows: "Submitted form: [form name]" ✅

Viewing Submissions
To see all submissions for a form:
  1. Go to Lead Capture → Forms
  2. Click the form name to open it
  3. Click the Submissions tab
Each submission row shows:
  • The contact name and whether they were a New contact or an Updated existing contact
  • The submitted field values (expandable)
  • The page URL the form was submitted from
  • The date and time of submission

Click the contact link in any row to go directly to that contact's detail page.

    Managing Your Forms
From the Forms list you can:

    Action

    How

    Edit

Click the form name to reopen the builder

    Toggle ON/OFF

Use the Active switch to pause a form without deleting it. Visitors see "Form unavailable" when it is off

    Get embed code

Click the < > icon to copy the embed script or direct link

    Delete

Click the delete icon — the form and its public URL are permanently removed

Note: Turning a form OFF means new submissions will be blocked with a "Form unavailable" message. Existing submissions and contacts are not affected.

    Connecting to Automations
Once your form is live, you can trigger automations whenever someone submits it using the Form Submitted trigger.
How the Form Submitted Trigger Works
When a visitor submits your form, myCRMSIM fires a Form Submitted event. This event carries the contact's data so your workflow can act on it immediately — tagging the contact, sending a follow-up SMS, adding them to a pipeline, and more.
Setting Up a Form Submitted Workflow
    1. Go to Automations in the left sidebar
    2. Click New Workflow
    3. In the trigger palette, find Form Submitted under Triggers
    4. Click the trigger to open its properties panel
    5. Choose one of two options:

    Option

    When to Use

    Specific form

Workflow fires only when the selected form is submitted — useful when different forms need different follow-up sequences

    Any form

Workflow fires for every form submission in your workspace

Add your actions after the trigger. Common examples:

    Action

    What it Does

    Add Tag

    Tag the contact (e.g. new-lead, form-submitted)

    Send SMS

Send an immediate follow-up message to the contact

    Add to Pipeline

Move the contact into a sales pipeline stage

    Assign to Team Member

Route the lead to a specific person

    Send Internal Notification

Alert your team that a new form lead came in

Click Save and make sure the workflow is set to Active

What Happens When Both Triggers Apply
If a visitor submits a form for the first time (new contact), two triggers can fire together:
    • Contact Created — fires because a new contact was created
    • Form Submitted — fires because a form was submitted
If the same visitor submits again (existing contact), Contact Updated fires instead of Contact Created, but Form Submitted fires again for the new submission.
If the Automations Backend is Unavailable
The Form Submitted trigger is fire-and-forget — if the automations service is temporarily unavailable, the visitor's contact and submission are still saved correctly. The trigger logs a warning but the form experience is never affected.
    How Contact Deduplication Works
myCRMSIM is smart about avoiding duplicate contacts:

    Scenario

    What Happens

    New email submitted

    New contact created

    Same email submitted again

Existing contact updated — no duplicate

    Same phone submitted (no email)

Matched by phone — existing contact updated

    Optional field left empty

Existing value on the contact is never overwritten by a blank submission

Email matches one contact, phone matches another

Matched by email (email takes priority)

    Frequently Asked Questions
    1. Can I have multiple forms for different pages?
Yes. Create a separate form for each page or use case. Each form gets its own embed code, tracks submissions independently, and can trigger a different automation workflow.
    2. Can I add custom fields to my form?
Yes. Any custom contact fields you have defined in your workspace (under Contacts → Custom Fields) will appear in the Add Field menu inside the form builder.
    3. What if a visitor submits without an email or phone number?
The form will not submit — at least one of Email or Phone Number is required on every form. This ensures every submission can be matched to a contact.
    4. Can I restrict the form to my website only?
Yes. Enter your domain in the Allowed websites field under Settings. Submissions from any other domain will be blocked with a 403 error.
    5. Does the form work on mobile?
Yes. The form is fully responsive and works on all screen sizes — single column layout, no horizontal scroll.
    6. Can I see where a submission came from?
Yes. The Submissions view records the page URL the form was submitted from, so you can see which page on your website is generating the most leads.
